Platelet-rich plasma combined with celecoxib in the treatment of patients with knee osteoarthritis
富血小板血浆联合塞来昔布治疗膝关节骨性关节炎患者

摘要

目的 探究富血小板血浆联合塞来昔布治疗KOA的效果。方法 选取2020年3月~2022年2月本院收治的KOA患者86例作为研究对象。塞来昔布治疗的患者纳入对照组,塞来昔布+富血小板血浆治疗的患者纳入实验组(各组43例)。比较2组的VAS评分、血清炎症因子水平、血清促生长相关因子水平、Lysholm膝关节评分、WOMAC评分、影像学图像、膝关节活动度、SF-36评分、治疗总有效率。结果 两组患者在治疗前上述的各项指标均无统计学差异(P>0.05),实验组在治疗完成时的VAS评分、WOMAC评分、血清炎症因子水平、关节腔积液范围、滑膜厚度、胫骨近端内侧角均小于对照组(P<0.05),血清促生长相关因子水平、Lysholm评分、关节软骨厚度、膝关节活动度、SF-36评分高于对照组(P<0.05)。对照组治疗前后的血清促生长相关因子水平无显著变化(P>0.05),其余指标治疗完成时有明显改善(P<0.05),但在之后逐渐向治疗前的状态变化(P<0.05);实验组的各项指标在治疗完成时均明显改善(P<0.05),且在之后无明显变化(P>0.05)。实验组的治疗总有效率高于对照组(P<0.05)。结论 相比于单纯使用塞来昔布治疗KOA,富血小板血浆联合塞来昔布治疗KOA能够增强抗炎、镇痛效果,且能够有效促进受损膝关节软骨组织修复,进一步改善膝关节组织结构、膝关节功能,提高患者的生活质量。

Abstract

Objective: To explore the effect of platelet-rich plasma combined with celecoxib in the treatment of KOA.
Methods: A total of 86 KOA patients admitted to our hospital from March 2020 to February 2022 were selected as the research subjects. The patients treated with celecoxib were included in the control group, and the patients treated with celecoxib + platelet-rich plasma were included in the experimental group (43 cases in each group). The VAS score, serum inflammatory factor levels, serum growth-promoting-related factor levels, Lysholm knee joint score, WOMAC score, imaging images, knee joint range of motion, SF-36 score, and total effective rate of treatment were compared between the two groups.
Results: There was no statistical difference in the above indicators between the two groups before treatment (P>0.05). The VAS score, WOMAC score, serum inflammatory factor level, joint effusion range, synovial membrane of the experimental group at the completion of treatment Thickness and medial angle of proximal tibia were lower than those in the control group (P<0.05), and the levels of serum growth-promoting factors, Lysholm score, articular cartilage thickness, knee range of motion and SF-36 score were higher than those in the control group (P<0.05). There was no significant change in the levels of serum growth-promoting factors in the control group before and after treatment (P>0.05), and other indicators were significantly improved after treatment (P<0.05), but gradually changed to the state before treatment (P<0.05); All indexes in the experimental group were significantly improved at the completion of treatment (P<0.05), and there was no significant change after that (P>0.05). The total effective rate of treatment in the experimental group was higher than that in the control group (P<0.05).
Conclusion  : Compared with celecoxib alone in the treatment of KOA, platelet-rich plasma combined with celecoxib in the treatment of KOA can enhance the anti-inflammatory and analgesic effects, and can effectively promote the repair of damaged knee cartilage tissue and further improve the knee joint tissue. structure, knee function, and improve the quality of life of patients.
