个案全程管理模式在慢性心力衰竭患者护理中的作用

摘要

目的 研究分析慢性心力衰竭患者护理中个案全程管理模式的表现。方法 本次实验跨越2021年1月至2021年12月这一时间段，研究人员对70例慢性心力衰竭患者进行研究，研究人员在2021年期间对所选慢性心力衰竭患者实施个案全程管理模式，采用回顾性分析法，记录个案全程管理模式实施前后所选患者自我管理评分、再入院情况、患者对护理工作所持态度，并实施比较分析。结果 干预前后所选患者自我管理评分相关数据加以比较分析，干预前后所选患者在症状管理、心理和社会适应、饮食管理及药物管理方面的评分均有着较为明显的差距，（p＜0.05）；干预前后所选患者再入院情况进行比较分析，干预前70例患者中再入院人数为26人，占比为37.14%，干预后对应人数为8人，占比为11.43%，患者再入院率相关数据差异较大，（p＜0.05）；干预前后所选患者对护理工作持十分认可、比较认可态度的人数分别为48人及66人，占比分别为68.57%及94.29%，比较分析护理认可度相关数据，有着较大的差距，（p＜0.05）。结论 在慢性心力衰竭患者护理中个案全程管理模式的应用价值较高。

Abstract

Objective To study and analyze the performance of the whole case management model in the nursing of patients with chronic heart failure.
Methods: This experiment spanned the period from January 2021 to December 2021. The researchers studied 70 patients with chronic heart failure. During 2021, the researchers implemented the case management model for the selected patients with chronic heart failure. , using retrospective analysis to record the selected patients' self-management scores, readmissions, and patients' attitudes toward nursing work before and after the implementation of the case management model, and conduct comparative analysis.
Results: The data about the self-management scores of the selected patients before and after the intervention were compared and analyzed. The scores of the selected patients before and after the intervention in symptom management, psychological and social adaptation, diet management and drug management were significantly different (p<0.05). The readmission of selected patients before and after the intervention was compared and analyzed. Among the 70 patients before the intervention, the number of readmissions was 26, accounting for 37.14%, and the corresponding number after the intervention was 8, accounting for 11.43%.
