Observation on the therapeutic effect of "Zhishui Hanmu" acupuncture combined with breathing guidance in patients with post-stroke depression and sleep disorders
“滋水涵木”针刺联合呼吸导引术干预卒中后抑郁伴睡眠障碍患者疗效观察

摘要

目的 探讨“滋水涵木”针刺法联合呼吸导引术对卒中后抑郁（PSD）患者抑郁情绪和睡眠质量的影响。方法 将符合纳入标准的80例卒中后抑郁伴睡眠障碍患者，按随机数字表法纳入对照组和干预组，各40例。对照组（脱落3例）给予脑卒中恢复期对症支持治疗+综合康复训练，干预组（脱落2例）在对照组的基础上采用“滋水涵木”针刺法+呼吸导引术。干预前后均采用汉密尔顿抑郁量表( HAMD-17)、匹兹堡睡眠质量量表(PSQI)评价疗效。结果 干预4周后，干预组患者的抑郁程度（HAMD-17）和睡眠质量PSQI总分及各维度分均较对照组明显降低，总有效率(92.11%)明显高于对照组(70.27%），差异有统计学意义（P＜0. 05）。结论 在脑卒中对症支持治疗+综合康复的基础上，采用“滋水涵木”针刺法联合呼吸导引术可有效改善患者的抑郁情绪、提高睡眠质量。

Abstract

Objective: To investigate the effect of "Zi Shui Han Mu" acupuncture combined with breathing guidance on depression and sleep quality in patients with post-stroke depression (PSD).
Methods A total of 80 patients with post-stroke depression and sleep disorders who met the inclusion criteria were included in the control group and the intervention group according to the random number table method, 40 cases in each group. The control group (3 cases dropped out) was given symptomatic and supportive treatment + comprehensive rehabilitation training during the recovery period of stroke, and the intervention group (2 cases dropped out) was given the "Zhishui Hanmu" acupuncture method + breathing guidance on the basis of the control group. The Hamilton Depression Scale (HAMD-17) and Pittsburgh Sleep Quality Inventory (PSQI) were used to evaluate the efficacy before and after the intervention.
Results After 4 weeks of intervention, the depression degree (HAMD-17) and sleep quality PSQI total score and each dimension score of the intervention group were significantly lower than those of the control group, and the total effective rate (92.11%) was significantly higher than that of the control group (70.27%). , the difference was statistically significant (P<0.05).
Conclusion   On the basis of symptomatic and supportive treatment of stroke + comprehensive rehabilitation, the use of "Zi Shui Han Mu" acupuncture combined with breathing guidance can effectively improve patients' depression and improve sleep quality.
