Anaesthesia effect of remifentanil in total hip arthroplasty
瑞芬太尼在全髋关节置换术中的麻醉作用分析

摘要

目的 探讨瑞芬太尼在全髋关节置换术中的麻醉作用。方法 选取我院自2019年6月到2021年6月期间收治的88例行髋关节置换术治疗者作为研究对象，将患者随机分为两组，对照组采用舒芬太尼进行维持麻醉，观察组采用瑞芬太尼进行维持麻醉，对两组患者的麻醉效果进行比较。结果 两组手术时间对比无较大差异（p>0.05），但观察组术后睁眼时间、语言恢复时间均短于对照组（p<0.05）；观察组拔管后疼痛程度（Prince-Henry）评分低于对照组、苏醒评分（Steward）高于对照组（p<0.05）；术前1d、术后1d、术后5d比较，两组患者认知功能（MMSE）评分均无较大差异（p>0.05）。结论 在全髋关节置换术中以瑞芬太尼进行维持麻醉可取得较好的效果，并且不会给患者术后认知功能带来明显影响，值得推广使用。

Abstract

Objective To investigate the anesthetic effect of remifentanil in total hip arthroplasty.
Methods: A total of 88 patients who underwent hip replacement in our hospital from June 2019 to June 2021 were selected as the study subjects, and the patients were randomly divided into two groups: the control group received sufentanil for maintenance anesthesia, and the observation group received remifentanil for maintenance anesthesia, and the anesthesia effect of the two groups was compared.
Results: There was no significant difference in the operative time between the two groups (P >0.05), but the postoperative eye opening time and language recovery time of the observation group were shorter than those of the control group (P <0.05). The prince-Henry score after extubation in the observation group was lower than that in the control group, and the Steward score was higher than that in the control group (P <0.05). There was no significant difference in cognitive function (MMSE) score between 2 groups 1 day before surgery, 1 day after surgery and 5 days after surgery (P >0.05).
Conclusion  : Remifentanil for maintenance anesthesia in total hip arthroplasty can achieve good results, and does not bring significant effects on postoperative cognitive function of patients, so it is worth popularizing.
