目的 比较布地奈德联合出生后肺泡表面活性物质(Pulmonary Surfactant, PS)使用和单独使用PS对早产儿支气管肺发育不良(Broncho Pulmonary Dysplasia, BPD)的防治效果。方法 选取2021年6月至2024年12月贵州省兴义市人民医院新生儿科收治的早产儿BPD患儿65例,以随机为基本原则,分为实验组(A组)与对照组(B组)。B组早期单纯予以牛肺表面活性剂治疗(70-100 mg/kg),在此基础上,A组在使用牛肺表面活性剂同时,加用布地奈德0.25mg/kg一同气道内滴入。结果 A组与B组的BPD总发生率虽无明显差异,但在患儿拔管时间、吸氧时间、住院时间方面A组优于B组,在血钙血磷、脑室内出血方面A组与B组差异无统计学意义。结论 布地奈德联合PS气道内滴入,通过降低早产儿BPD严重程度,缩短早产儿吸氧时间、住院时间,起到治疗早产儿BPD的效果。
Objective To compare the preventive and therapeutic effects of using budesonide in combination with PS and using PS alone on bronchopulmonary dysplasia (BPD) in premature infants. Methods Sixty-five premature infants with BPD who were admitted to the Neonatal Department of Xingyi People's Hospital in Guizhou Province from June 2021 to December 2024 were selected. Based on the principle of randomization, they were divided into the experimental group (Group A) and the control group (Group B). Group B was treated with bovine pulmonary surfactant alone at a dose of 70-100mg/kg in the early stage. On this basis, Group A was given budesonide at a dose of 0.25 mg/kg in addition to the bovine pulmonary surfactant and instilled into the airway. Results Although there was no significant difference in the total incidence of BPD between Group A and Group B, Group A had better outcomes in terms of extubation time, oxygen inhalation time, and hospital stay compared to Group B. There was no statistically significant difference in blood calcium and phosphorus levels, or intraventricular hemorrhage between Group A and Group B. Conclusion The combined use of budesonide and PS instillation into the airway can reduce the severity of BPD in premature infants, shorten the oxygen inhalation time and hospital stay of premature infants, and achieve the therapeutic effect of BPD in premature infants.
