目的 本研究旨在探讨经尿道钬激光前列腺剜除术(HoLEP)作为日间手术(SDS)的安全性、可行性和经济学优势。方法 前瞻性随机对照研究于2023年9月至2024年9月期间在眉山市中医医院泌尿外科进行HoLEP手术的符合纳入标准的200例患者的临床数据及资料。由同一术者完成HoLEP手术治疗。根据治疗住院的流程及围手术期的不同分为SDS日间HoLEP组(SDS组)和普通HoLEP组(非SDS组),比较两组间的患者的人口统计学资料临床特征数据和治疗随访结果。结果 两组间的患者的人口统计学资料无明显统计学差异。两组在术后均顺利拔除尿管,未出现再入院情况。SDS组在住院时间、留置导尿时间和住院费用、病人满意度方面均优于非SDS组。术后3个月随访结果显示,两组患者的生活质量评分(QOL)、国际前列腺症状评分(IPSS)及最大尿流率(Qmax)均较术前显著改善,且改善幅度方面两组间无显著差异。结论 HoLEP在SDS模式下具有安全、经济、康复快、满意度高等优势,为泌尿外科日间手术模式的推广提供了可行依据。
Objective The aim of this study was to investigate the safety, feasibility, and therapeutic efficacy of Holmium Laser Enucleation of the Prostate (HoLEP) as Same Day Surgery (SDS). Methods A prospective randomized controlled study was conducted from September 2023 to September 2024 at the Department of Urology, Meishan Traditional Chinese Medicine Hospital, involving clinical data and materials from 200 eligible patients undergoing HoLEP surgery. All HoLEP procedures were performed by the same surgeon. The patients were categorized into SDS HoLEP group (SDS group) and normal HoLEP group (non-SDS group) according to the different methods of perioperative management, and the patients' demographic information clinical characteristics data and treatment follow-up results were compared between the two groups. Results There was no statistically significant difference in the demographic data of the patients between the two groups. In both groups, urinary catheters were successfully removed after surgery without readmission, and the SDS group was better than the non-SDS group in terms of hospitalization time, indwelling catheterization time and hospitalization cost, and patient satisfaction. At the follow-up visit after 3 months, the preoperative and postoperative maximal urinary flow rate (Qmax), Quality of Life(QOL), and International Prostate Symptom Score (IPSS) of the two groups had significantly improved, and the magnitude of the improvement did not differ significantly between the two groups. Conclusion HoLEP offers significant advantages in SDS mode, including safety, cost-effectiveness, rapid recovery, and high patient satisfaction, providing a viable basis for promoting the day surgery model in urology.
