目的 分析基于信息化技术的居家糖尿病护理平台联合精准护理模式对糖尿病患者血糖控制水平及生活质量的影响。方法 选取某院2024年6月至2025年6月期间的糖尿病患者130例，随机分为干预组与对照组。对照组接受常规出院指导与门诊随访，干预组在此基础上应用居家糖尿病护理平台并实施精准护理。结果 干预组的HbA1c、FPG、2hPG水平均明显低于对照组（P<0.05）。在生活质量评估中，干预组DSQL总分更低（P<0.05）。结论 将居家糖尿病护理平台与精准护理策略相结合，能够有效提升糖尿病患者的血糖管理效果。
Objective To analyze the impact of a home-based diabetes care platform combined with precision nursing on blood glucose control levels and quality of life in diabetic patients. Methods A total of 130 diabetic patients from a certain hospital between June 2024 and June 2025 were selected and randomly divided into an intervention group and a control group. The control group received routine discharge guidance and outpatient follow-up, while the intervention group additionally utilized the home-based diabetes care platform and implemented precision nursing. Results The intervention group showed significantly lower levels of HbA1c, FPG, and 2hPG compared to the control group (P<0.05). In quality of life assessments, the intervention group had a lower DSQL total score (P<0.05). Conclusion Combining a home-based diabetes care platform with precision nursing strategies can effectively improve blood glucose management outcomes in diabetic patients.
