A study on the pain control effect of trans-theoretical model combined with case management-based health records in patients with advanced gastric cancer and breakthrough pain
跨理论模型联合个案管理式健康档案对晚期胃癌爆发痛患者疼痛控制效果的研究

摘要

目的 探讨跨理论模型联合个案管理式健康档案对晚期胃癌爆发痛患者疼痛控制效果。方法 选择2024年10月-2025年12月期间我院收治的晚期胃癌爆发痛患者38例，按照随机分组的方式分为对照组和实验组，对照组采取常规护理，实验组实施跨理论模型联合个案管理式健康档案护理干预。对比两组患者疼痛控制效果及护理满意度。结果 实验组患者干预后的24小时内爆发痛发作频率、单次爆发痛持续时间均优于对照组（P＜0.05）。实验组患者的护理满意度优于对照组（P＜0.05）。结论 跨理论模型联合个案管理式健康档案护理干预可有效降低晚期胃癌爆发痛患者24小时内爆发痛发作频率和单次持续时间，提高患者护理满意度。

Abstract

Objective To explore the pain control effects of the Transtheoretical Model combined with case-managed health records in patients with advanced gastric cancer experiencing breakthrough pain.
Methods Thirty-eight patients with advanced gastric cancer and breakthrough pain admitted to our hospital from October 2024 to December 2025 were randomly divided into a control group and an experimental group. The control group received routine nursing care, while the experimental group received nursing intervention based on the Transtheoretical Model combined with case-managed health records. The pain control effects and nursing satisfaction were compared between the two groups.
Results In the experimental group, the frequency of breakthrough pain episodes and the duration of single episodes within 24 hours after intervention were superior to those in the control group (P < 0.05). The nursing satisfaction of the experimental group was also better than that of the control group (P < 0.05).
Conclusion   Nursing intervention combining the Transtheoretical Model and case-managed health records can effectively reduce the frequency of breakthrough pain episodes and the duration of single episodes within 24 hours in patients with advanced gastric cancer, while improving patient satisfaction with nursing care.
