目的 研究聚多卡醇注射的多面体蜂窝结构泡沫硬化剂在内痔粘膜的作用机制和临床止血效果。方法 选择我科2024年03月01日至2024年12月31日期间我科收住的内痔出血导致重度贫血的63例患者,所有患者均因内痔出血导致重度贫血(即入院时血红蛋白小于60g/L),对该63例内痔出血的患者在纠正重度贫血的同时,使用聚多卡醇泡沫硬化剂,注射于内痔洞状血管区,观察患者注射后不同时间的大便出血情况。结果 3%聚多卡醇注射液与空气按照1:4配置成多面体蜂窝结构泡沫硬化剂,治疗内痔出血的有效率100%,肛门镜下观察内痔洞状血管区内痔粘膜纤维角化明显。结论 聚多卡醇注射液治疗内痔出血效果显著,其在安全性和易操作性值得临床大力推广。
Objective To investigate the mechanism of action and clinical hemostatic efficacy of a polyhedral honeycomb-structured polidocanol foam sclerosant in the management of internal hemorrhoids. Methods A total of 63 patients with severe anemia (defined as hemoglobin levels <60 g/L upon admission) due to bleeding internal hemorrhoids, admitted to our department between March 1, 2024, and December 31, 2024, were enrolled. While undergoing correction of severe anemia, all patients were treated with polidocanol foam sclerosant injected into the cavernous body area of the internal hemorrhoids. Post-injection rectal bleeding was monitored at various time points. Results The polyhedral honeycomb-structured foam sclerosant, prepared at 1:4 ratio using 3% polidocanol and air, achieved an efficacy rate of 100% in treating hemorrhoidal bleeding. Anoscopic examination revealed significant fibrosis and keratinization of the hemorrhoidal mucosa within the cavernous body area. Conclusion Polidocanol foam sclerotherapy is highly effective in the treatment of bleeding internal hemorrhoids. Its favorable safety profile and ease of operation support its widespread clinical application.
