Perioperative case report of a child with pneumonia and Pierre Robin sequence based on evidence-based evidence undergoing mandibular distraction osteogenesis
一例基于循证的皮罗序列征合并肺炎患儿行下颌骨牵引成骨术的围术期个案报告

摘要

分析一例皮罗序列征患儿下颌骨牵引成骨术个案护理。皮罗序列征（Pierre Robin sequence, PRS）患儿因先天性气道畸形易并发肺炎，围术期管理难度显著增加。本文通过多学科协作模式，运用“五维高级健康评估”框架（疾病与病症、整体健康、生理功能、自理能力、风险与并发症），制定循证护理方案。术中关键干预包括：气道体位管理、感染预防、术中精准体温保护、体液管理。结果显示：患儿术后5天撤离呼吸机，10天实现全口喂养，顺利出院，未发生误吸及切口感染。本案例证实系统性围术期管理对复杂PRS患儿的临床价值，为罕见病护理标准化提供实践参考。

Abstract

Analysis of Nursing Care for a Case of Mandibular Distraction Osteogenesis in a Child with Pierre Robin Sequence. Children with Pierre Robin Sequence (PRS) are prone to pneumonia due to congenital airway malformations, which significantly increases the difficulty of perioperative management. This study adopted a multidisciplinary collaboration model and utilized the “Five-Dimensional Advanced Health Assessment” framework (diseases/conditions, holistic health, physiological function, self-care capacity, risks/complications) to develop an evidence-based care plan. Key interventions include: airway and positioning management, infection prevention, intraoperative precise temperature protection, and fluid management.
Results demonstrated successful ventilator weaning on postoperative day 5, full oral feeding achieved by day 10, with zero aspiration events or surgical site infections. This case confirms the clinical value of systematic perioperative management for complex PRS patients and provides practical references for standardizing rare disease care.
