Research on the medication laws of Wumen medical school in treating chronic dysentery based on collateral disease theory: A data mining analysis
基于数据挖掘的吴门医派运用络病理论治疗久痢用药规律研究

摘要

目的 运用数据挖掘技术，分析吴门医派医家在络病理论指导下治疗久痢的处方用药规律与组方特征，探讨其学术思想及辨治内涵。方法 检索《中华医典》及相关馆藏古籍，筛选吴门医派医家运用络病理论治疗久痢的医案处方。规范化处理后，采用中医传承计算平台V3.5进行药物频次、性味、归经统计，并运用关联规则及聚类分析挖掘高频药对、核心药物组合及组方规律。结果 共纳入处方67首，药物117味，总使用频次559次。高频药物前五位依次为茯苓、当归、白芍、甘草、炮姜；药性以温性为主；药味以甘味、苦味、辛味为多。归经聚焦于脾经、肾经、肝经。关联规则分析获得“当归-白芍”、“茯苓-白芍”等13个高频药对，以及“泽泻→茯苓”等强关联规则。聚类分析得到4类核心药物组合，分别体现了“寒温并用，气血兼顾”、“温阳化气，养血通络”、“寒热互济，辛开苦降”及“脾肾肝同补”的治法。结论 吴门医派治疗久痢紧扣久病入络病机，以通络为纲，临证重视温补脾肾、养血通络，形成了虚实兼顾、标本同治的辨治思想。

Abstract

Objective Using data mining technology to analyze the prescription medication rules and prescription characteristics of Wumen medical school doctors in the treatment of chronic dysentery under the guidance of collateral disease theory, and to explore its academic thought and connotation of syndrome differentiation and treatment.
Methods Retrieve‘ Chinese Medical Classics’ and related ancient books, and screen the prescriptions of Wumen medical doctors using collateral disease theory to treat chronic dysentery. After standardized processing, the frequency, taste and meridian tropism of drugs were counted by using the traditional Chinese medicine inheritance calculation platform V3.5, and the association rules and cluster analysis were used to mine high-frequency drug pairs, core drug combinations and prescription rules.
Results A total of 67 prescriptions and 117 drugs were included, with a total frequency of 559 times. The top five high-frequency drugs were Poria cocos, Angelica sinensis, Paeoniae Radix Alba, Glycyrrhizae Radix et Rhizoma and Paojiang. The medicinal properties were mainly warm. The main flavors were sweet, bitter and pungent. The meridian tropism focused on the spleen meridian, kidney meridian and liver meridian. Through association rule analysis, 13 high-frequency herb pairs such as ‘ Danggui-Baishao ’, ‘ Fuling-Baishao ’, and strong association rules such as ‘Zexie → Fuling’ were obtained. Four kinds of core drug combinations were obtained by cluster analysis, which respectively reflected the treatment methods of cold and warm combination, qi and blood balance’, Warming yang and transforming qi, nourishing blood and dredging collaterals ’, ‘ cold and heat mutual aid, pungent opening and bitter descending ’ and ‘ spleen, kidney and liver tonic ’.
Conclusion   The treatment of chronic dysentery by Wumen medical school is closely related to the pathogenesis of chronic disease entering the collaterals. With the dredging of collaterals as the key link, the clinical attention is paid to warming and tonifying the spleen and kidney, nourishing blood and dredging collaterals, forming the thought of syndrome differentiation and treatment of both deficiency and excess, and treating both symptoms and root causes.
