救心丸治疗阵发性心房颤动合并高血压的临床研究

摘要

目的 探讨救心丸治疗阵发性心房颤动合并高血压患者的临床疗效及安全性。方法 选取自2022年1月5日至2023年12月18日在本院接受治疗的268例阵发性心房颤动合并高血压患者,随机分为对照组(n=134)与研究组(n=134)。两组均给予常规抗高血压及抗心律失常治疗,研究组在此基础上加用救心丸口服治疗,一次1粒,每日2次,疗程为4周。观察并比较两组治疗前后心房颤动发作频率、发作持续时间、24小时动态心电图房颤负荷、左心房内径及高敏C反应蛋白(hs-CRP)水平变化,记录不良反应发生情况。结果 治疗4周后,研究组心房颤动发作频率显著低于对照组(3.21±1.04次/周vs5.47±1.26次/周,P<0.001),发作持续时间及房颤负荷显著减少(P<0.01),左心房内径缩小(P=0.034),hs-CRP水平下降更为显著(P=0.015)。治疗期间两组患者均未出现明显不良反应,未见心悸加重、头晕、胃肠不适等药物相关不良事件,整体耐受性良好。结论 在常规治疗基础上联合救心丸可显著降低阵发性心房颤动合并高血压患者的房颤发作频率与炎症水平,改善心房结构重塑,且安全性良好,具有较高的临床应用价值。此外,研究组患者治疗后汉密尔顿焦虑量表(HAMA)和抑郁量表(HAMD)评分显著降低,提示救心丸在缓解焦虑、抑郁方面亦具有良好作用。

Abstract

Objective To investigate the clinical efficacy and safety of Jiuxin Pill in treating patients with paroxysmal atrial fibrillation complicated with hypertension.
Methods 268 patients with paroxysmal atrial fibrillation complicated with hypertension treated in our hospital from January 5, 2022 to December 18, 2023 were randomly divided into a control group (n=134) and a study group (n=134). Both groups received routine antihypertensive and antiarrhythmic treatment. The study group received additional oral Jiuxin Pill treatment, one pill twice daily, for 4 weeks. Changes in atrial fibrillation frequency, duration, 24-hour Holter monitoring atrial fibrillation burden, left atrial diameter, and high-sensitivity C-reactive protein (hs-CRP) levels before and after treatment were observed and compared between the two groups. Adverse reactions were also recorded.
Results After 4 weeks of treatment, the frequency of atrial fibrillation episodes in the study group was significantly lower than that in the control group (3.21±1.04 times/week vs 5.47±1.26 times/week, P<0.001), the duration of episodes and atrial fibrillation burden were significantly reduced (P<0.01), the left atrial diameter decreased (P=0.034), and the hs-CRP level decreased more significantly (P=0.015). No significant adverse reactions occurred in either group during the treatment period; no drug-related adverse events such as worsening palpitations, dizziness, or gastrointestinal discomfort were observed, and overall tolerability was good.
Conclusion   Combining Jiuxin Pills with routine treatment can significantly reduce the frequency of atrial fibrillation episodes and inflammation levels in patients with paroxysmal atrial fibrillation and hypertension, improve atrial structural remodeling, and has good safety, demonstrating high clinical application value. Furthermore, the Hamilton Anxiety Rating Scale (HAMA) and Hamilton Depression Rating Scale (HAMD) scores of patients in the study group significantly decreased after treatment, suggesting that Jiuxin Pills also have a good effect in relieving anxiety and depression.
