The effect of continuity nursing model on enhancing home self-management ability in hemodialysis patients
延续性护理模式在血液透析患者居家自我管理能力提升中的应用效果

摘要

目的 探究在血液透析患者居家自我管理能力提升中，采用延续性护理模式的效果。方法 选取本院血液透析患者86例，时间为2024年4月—2025年4月，随机分组，对照组43例，在患者居家自我管理能力提升中，采用常规护理，观察组43例，采用延续性护理模式，对比两组患者护理效果。结果 观察组居家自我管理能力各维度评分，高于对照组，并发症发生率，低于对照组，差异有统计学意义（P＜0.05）。结论 在血液透析患者护理中应用延续性护理模式，可有效提升患者居家自我管理能力，控制并发症发生风险，值得推广。

Abstract

Objective To explore the effect of adopting a continuity of care model in improving the self-management ability of hemodialysis patients at home.
Methods 86 hemodialysis patients from our hospital were selected from April 2024 to April 2025 and randomly divided into two groups: a control group of 43 patients who received routine nursing care to improve their self-management ability at home, and an observation group of 43 patients who received continuous nursing care. The nursing effects of the two groups were compared.
Results The observation group had higher scores in various dimensions of self-management ability at home than the control group, and lower incidence of complications than the control group, with statistically significant differences (P<0.05).
Conclusion   The application of continuity of care model in the care of hemodialysis patients can effectively improve their self-management ability at home, control the risk of complications, and is worthy of promotion.
