家庭医生签约服务在慢病管理中的作用研究

摘要

目的 研究家庭医生签约服务在慢病管理中的作用，为临床开展家庭医生签约服务提供参考。方法将2024年2月-2025年2月接收的110例慢病患者纳入研究，随机分为试验组与参考组，每组55例。参考组给予传统护理服务，试验组给予家庭医生签约服务，对比两组自我管理、并发症评分和住院频率。结果 试验组自我管理（坚持用药、健康饮食、定期监测）评分高于参考组（P＜0.05）；试验组并发症评分、住院频率低于参考组（P＜0.05）。结论 家庭医生签约服务在慢病管理中有显著的积极作用，能够提高患者自我管理能力，减少并发症和住院次数，可作为临床科学管理慢病患者的一种有效方式。

Abstract

Objective To study the role of family doctor contract services in chronic disease management and provide reference for the clinical implementation of family doctor contract services.
Methods 110 chronic disease patients received from February 2024 to February 2025 were included in the study and randomly divided into an experimental group and a reference group, with 55 patients in each group. The reference group received traditional nursing services, while the experimental group received family doctor contract services. The self-management, complication score, and hospitalization frequency of the two groups were compared.
Results The self-management scores of the experimental group (adherence to medication, healthy diet, regular monitoring) were higher than those of the reference group (P<0.05); The complication score and hospitalization frequency of the experimental group were lower than those of the reference group (P<0.05).
Conclusion   Family doctor contract services have a significant positive effect on chronic disease management, which can improve patients' self-management ability, reduce complications and hospitalization times, and can be an effective way for clinical scientific management of chronic disease patients.
