Analysis of the effectiveness of community care in elderly patients with coronary heart disease and type 2 diabetes mellitus
社区护理在老年冠心病合并2型糖尿病患者中的应用效果分析

摘要

目的 浅析老年冠心病合并2型糖尿病患者采用社区护理指导，为慢性病患者的健康生活、并发症预防提供建议。方法 选取2024年1月至2024年12月收治的老年冠心病合并2型糖尿病患者100例，对照组采用基础护理；观察组采用社区护理，对比临床护理效果。结果 观察组慢性病患者自我护理能力评分高于对照组，对比差异显著（P＜0.05）。此外，观察组患者慢性病相关知识知晓率高于对照组，对比差异显著（P＜0.05）。最后，观察组患者遵医行为高于对照组，对比差异显著（P＜0.05）。结论 针对老年冠心病合并2型糖尿病患者实施社区护理的效果显著，不仅提升了患者的疾病知识知晓率和自我护理能力，也切实提高了患者的遵医行为，干预效果确切。

Abstract

Objective To analyze the elderly patients with coronary heart disease and type 2 diabetes by using community nursing guidance to provide suggestions for healthy life and complication prevention for patients with chronic diseases.
Methods 100 elderly patients with type 2 diabetes admitted from January 2024 to December 2024 were selected with basic nursing for the control group; community nursing in the observation group to compare the clinical nursing effect.
Results The self-care ability score of chronic patients was higher than the control group, with significant difference (P <0.05). In addition, the knowledge of chronic diseases was higher than that of the control group, was significant (P <0.05). Finally, the observation group showed higher compliance than the control group, with significant differences (P <0.05).
Conclusion   The effect of community nursing for elderly patients with type 2 diabetes is remarkable, which not only improves the disease knowledge awareness rate and self-care ability of patients, but also effectively improves the patients' medical compliance behavior, and the intervention effect is accurate.
