Clinical effect analysis of laparoscopic combined with cholangioscopy or duodenoscopy in the simultaneous treatment of gallstones with extrahepatic bile duct stones
腹腔镜联合胆道镜同期治疗胆结石合并肝外胆管结石的临床效果分析

摘要

目的 观察分析腹腔镜联合胆道镜同期治疗胆结石合并肝外胆管结石的临床效果和应用价值。方法采用回顾性研究方法，选择院内就诊的60例胆结石合并肝外胆管结石患者（样本纳入例数：60例；样本纳入时间：2023年5月至2024年5月），以随机摸球为基础分组方式，分为对照、实验两个小组，观察例数高度一致。分别实施腹腔镜治疗，腹腔镜联合胆道镜治疗。收集分析治疗效果。结果 实验组术中出血量更低，手术时间更长，术后下床时间、住院时间更短，差异具有统计学意义（P＜0.05）；胃功能恢复时间、不良反应发生率差异不显著（P＞0.05）；实验组手术后的生活质量评分（角色功能/情绪功能/认知功能/社会功能/躯体功能/总体健康）更高（P＜0.05）。结论 腹腔镜联合胆道镜同期治疗均可以有效缓解症状，控制胆结石合并肝外胆管结石患者病情，促进恢复，住院时间更短。

Abstract

Objective To observe and analyze the clinical effect and application value of laparoscopy combined with choledochoscopy in the simultaneous treatment of gallstones combined with extrahepatic bile duct stones.
Methods A retrospective study method was used to select 60 patients with gallstones and extrahepatic bile duct stones who were treated in the hospital (number of sample included: 60 cases; sample included time: May 2023 to May 2024), and randomly touched balls. As a basic grouping method, it is divided into two groups: control group and experimental group, and the number of observed cases is highly consistent. Laparoscopic treatment and combined laparoscopy and choledochoscopy were performed respectively. Collect and analyze treatment effects.
Results The experimental group had lower intraoperative blood loss, longer operation time, shorter postoperative out-of-bed time, and shorter hospitalization time. The differences were statistically significant (P<0.05); there was no significant difference in gastric function recovery time and incidence of adverse reactions. (P＞0.05); the quality of life score (role function/emotional function/cognitive function/social function/physical function/overall health) of the experimental group after surgery was higher (P＜0.05).
Conclusion   Laparoscopy combined with choledochoscopy can effectively relieve symptoms, control the condition of patients with gallstones and extrahepatic bile duct stones, promote recovery, and shorten the hospitalization time.
