The difference of therapeutic effect between artificial joint replacement and internal fixation in the treatment of osteoporotic unstable intertrochanteric fracture of femur
人工关节置换与内固定治疗骨质疏松性不稳定股骨粗隆间骨折的疗效差异研究

摘要

目的 分析人工关节置换和内固定治疗用于骨质疏松性不稳定股骨粗隆间骨折的价值。方法 随机均分2022年1月-2023年12月本科接诊骨质疏松性不稳定股骨粗隆间骨折病人（n=68）。试验组采取人工关节置换术，对照组行内固定治疗。对比Harris评分等指标。结果 关于Harris评分：治疗结束时，试验组的数据达到了（89.32±2.14）分，而对照组数据则仅有（81.73±3.05）分，相比较下，试验组的髋关节功能更好（P＜0.05）。关于住院时间卧床时间：试验组数据优于对照组（P＜0.05）。并发症发生率：试验组低至2.94%，而对照组数据则达到了17.65%，比较发现：试验组的发生率更低（P＜0.05）。VAS评分：治疗后，试验组低于（1.52±0.53）分，但对照组却达到（2.62±0.81）分，比较发现：试验组数据更低（P＜0.05）。疗效优良率：试验组达到97.06%，但对照组却仅有79.41%，比较发现：试验组数据更高（P＜0.05）。结论 骨质疏松性不稳定股骨粗隆间骨折用人工关节置换术，病人的卧床时间更短，病情恢复更快，并发症发生率更低，疼痛感更轻，疗效更好，髋关节功能改善也更加明显。

Abstract

Objective To analyze the value of artificial joint replacement and internal fixation in the treatment of osteoporotic unstable intertrochanteric fractures of femur.
Methods Patients with osteoporotic unstable intertrochanteric fracture of femur were randomly assigned from January 2022 to December 2023 (n=68). The experimental group underwent artificial joint replacement and the control group underwent internal fixation. Harris score and other indicators were compared.
Results The Harris score was 89.32±2.14 in the experimental group and 81.73±3.05 in the control group at the end of treatment. The hip function of the experimental group was better than that of the control group (P < 0.05). The length of stay in bed: the experimental group was better than the control group (P < 0.05). The incidence of complications was as low as 2.94% in the experimental group and 17.65% in the control group. It was found that the incidence of complications in the experimental group was lower (P < 0.05). VAS score: After treatment, the experimental group was lower than (1.52±0.53) points, but the control group was higher than (2.62±0.81) points, and the experimental group was lower (P < 0.05). The rate of excellent and good curative effect was 97.06% in the experimental group, but only 79.41% in the control group. It was found that the data in the experimental group was higher (P < 0.05).
Conclusion   Arthroplasty for osteoporotic unstable intertrochanteric fracture of femur can result in shorter bed time, faster recovery, lower complication rate, less pain, better efficacy and more obvious improvement in hip function.
