颈椎后路内镜Key-hole手术的腋下型入路治疗神经根型颈椎病

摘要

目的 探讨颈椎后路内镜Key-hole手术腋下型入路减压治疗单节段颈椎间盘突出症的临床可行性。方法 回顾性分析2015年6月至2020年9月本院骨科收治的86例符合纳入标准的病人,按术式分组对比患者的手术时间、减压面积、术前及术后末次的颈椎VAS、JOA及改良Macnab指数分析评估疗效。结果 患者均顺利完成手术。患者术后末次随访时临床症状得到明显改善,临床评分两组差异无统计学意义(P>0.05)。“腋下入路”组的患者磨骨开窗减压范围低于传统组,差异有统计学意义(P<0.05)。结论 Key-hole手术腋下型入路减压治疗单纯间盘突出的神经根型颈椎病可以达到治疗预期,缩短手术时间,并可较多保留关节突关节维持颈椎稳定性。

Abstract

Objective To explore the clinical feasibility of posterior endoscopic Key-hole surgery for axillary decompression in the treatment of single segmental cervical disc herniation.
Methods After 86 patients were included, they were divided into groups according to the mode of operation, and the relevant clinical indexes were evaluated.
Results There was no significant difference in preoperative indexes, but all patients were relieved after operation, and there was significant difference in treatment effect. There was no significant difference in postoperative curative effect between the two groups.
Conclusion   The subaxillary approach for Key-hole is effective.
